Eligibility Guidelines

Refer to Samaritan in the presence of the following indicators:

• Your patient has been diagnosed with one or more chronic conditions and is experiencing mobility issues which make traveling to medical appointments difficult.
• Your patient’s health is progressively deteriorating and not responding to curative treatments or disease management.
• Your patient has been diagnosed with a terminal illness or a condition with limited life expectancy.
• Your patient is requiring frequent hospitalizations for symptom management or is experiencing severe discomfort or poorly controlled symptoms that require specialized palliative expertise.
• Your patient has become increasingly dependent on assistance with the activities of daily living.
• Your patient has experienced a decline in physical or cognitive function which has impacted their quality of life.
• Life expectancy is six months or less if the disease follows its natural course.
