In collaboration with other physicians on your healthcare team, our board-certified physicians and palliative-trained nurse practitioners provide consultations in senior living communities, assisted living facilities, outpatient offices, select hospitals, or via telehealth. A patient who is deemed medically homebound and cannot come to the office setting can be seen in their home.
